Future credit line increases may be granted based on your overall credit performance.
ANNUAL MEMBERSHIP FEE: Credit Line $200: If your account is established with a credit line of $200, an Annual Membership Fee of $50 will be billed to your account when it's opened. Credit Line $300: If your account is established with a credit line of $300, an Annual Membership Fee of $75 will be billed to your account when it's opened. Credit Line $400: If your account is established with a credit line of $400, an Annual Membership Fee of $100 will be billed to your account when it's opened. Credit Line $500: If your account is established with a credit line of $500, an Annual Membership Fee of $125 will be billed to your account when it's opened. Credit Line $1,000: If your account is established with a credit line of $1,000, an Annual Membership Fee of $150 will be billed to your account when it's opened.
THE ANNUAL MEMBERSHIP FEE IS REFUNDABLE: As long as you cancel your account and have not used your card for any Purchases or Cash Advances and you have not made a payment.
ELIGIBILITY CRITERIA: The Bank may not extend credit to you if the information provided in this credit card application is not complete, accurate or verifiable and/or if you have an open or unpaid Credit One Bank account or a pending Credit One Bank application. If you are a resident of Wisconsin or a U.S. Territory, or have an APO/FPO address, you are not eligible for this program.
AGREEMENT: In this Agreement the words "I," "me," "my," "you," and "your" mean the applicant. "Bank," "we," and "us" mean Credit One Bank, N.A., located in Las Vegas, Nevada, its successors or assigns. I certify that I am 18 years of age or older and that the information in this credit card application is true and correct and is given in order to obtain credit. I authorize the Bank to check my credit report and to verify and exchange credit information about me with third parties, including credit bureaus, from time to time. I understand proof of income or other information may be requested from me or others. I understand I must provide a valid Social Security number. I agree to provide additional information as requested by the Bank from time to time. All information is subject to verification. The Bank may retain such credit or other information whether or not I receive a card. If this application is accepted and one or more credit cards are issued, I agree to pay all charges incurred in accordance with the Disclosures and Terms and Conditions in this Application, and the Cardholder Agreement, Disclosure Statement and Arbitration Agreement (and any future amendments), which will be sent with my card. I understand that once my credit card account is opened, it will be subject to the terms and conditions of the Cardholder Agreement, Disclosure Statement and Arbitration Agreement sent with my card, which may be amended from time to time and allows the Bank, at its sole discretion, to change all terms and conditions of the account upon notice to me in accordance with applicable law. I further acknowledge that the account will be for personal use and may not be used for business purposes.
IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT: The Bank complies with section 326 of the USA PATRIOT Act. This law mandates that we verify certain information about you while processing your account application.
ARBITRATION AGREEMENT: You and we agree that either you or we may, without the other's consent, require that any dispute between you and us be submitted to mandatory, binding arbitration. A more detailed description of this arbitration agreement will be sent with your card.
TREATMENT OF CHECK PAYMENTS: When you send us a personal check, you authorize us to process the check electronically. In addition, if a check we process in paper form is returned to us by your bank unpaid, we may re-present the returned check electronically. We use information from your check to electronically withdraw funds from your account. Funds may be withdrawn as soon as the business day we receive your payment. Electronically processed checks (and checks re-presented electronically) are not returned to you by your bank. However, Credit One Bank retains an image of your electronically processed check in accordance with regulatory record retention requirements. Credit One Bank will provide you with a copy of your electronically processed check(s) upon request.
STATE NOTICES: Residents of New York, Vermont, and Maine - A credit report may be obtained at any time for any legitimate purpose associated with the account including but not limited to reviewing, modifying, renewing, and collecting on your account. Upon your request, you will be informed whether or not a credit report was ordered, and if it was, you will be given the name and address of the credit bureau that furnished the report. New York residents may contact the New York State Banking Department to obtain a comparative listing of credit card rates, fees and grace periods. New York State Banking Dept. 1-800-522-3330. Residents of Ohio - Ohio laws against discrimination require that credit be equally available to all worthy customers and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio Civil Rights Commission administers compliance with this law. Residents of California - A married person may apply for a separate account.

Refer to the Features and/or Additional Benefits section of your card offer for the specific benefits included with your credit card account. CREDIT PROTECTION PROGRAM (“PROGRAM”) DISCLOSURES:
This Product Is Optional: Your purchase of the Credit Protection Program (“Program”) is optional. Whether or not you purchase the Program will not affect your application for credit or the terms of any existing credit agreement you have with the Bank.
Explanation of the Program Agreement: The Program cancels the Payment Due for the Current Month, which is defined below. The Credit Protection Agreement (“Agreement”) describes the specific circumstances under which the Payment Due for the Current Month may be cancelled up to a maximum of (6) payments if the primary cardholder becomes involuntarily unemployed or disabled. In the event of the primary cardholder’s death, the Account may be eligible for balance cancellation, up to a maximum of $10,000.
“Payment Due for the Current Month” is not a term used on the billing statement. Payment Due for the Current Month is the Amount Due This Period, as shown on the billing statement, less any Over Limit Fee and/or Late Payment Fee for the current billing cycle. Payment Due for the Current Month is the least amount that must be paid to remain current or, if your Account is past due at the commencement of the Benefit Period, the least amount that must be paid to remain at that same stage of delinquency. Only the Payment Due for the Current Month is eligible for cancellation. Amounts past due, Late Payment Fees, and Over Limit Fees included in the Minimum Payment Due, as shown on your monthly billing statement, are not included in the Payment Due for the Current Month and will not be cancelled. See Cardholder Agreement for information on the calculation of these billing statement amounts.
Amount of Program Charge: The monthly Program Charge is based on your Account balance each month multiplied by the unit-cost, which is $0.96 (96 cents) per $100, or part thereof.
Use of Card or Credit Line Restricted: If the Program is activated, your account will be closed and you will be unable to incur additional charges on the credit card or use the credit line while you are receiving benefits.
Termination of the Program: You have the right to cancel the Program at any time, for any reason. Program Charges assessed while you are in the Program will not be refunded.
The Bank has the right to cancel the Program, without notice, at any time and for any reason including, but not limited to, the following circumstances: if your Account is 60 days past due, your Account is over limit by 20% or more, you file for Bankruptcy, we determine or have a reasonable belief that you have committed fraud on the Account, or you are approved for participation in one of our debt management programs.
Eligibility Requirements, Conditions and Exclusions: There are eligibility requirements, conditions, and exclusions that could prevent you from receiving benefits under the Program. You may find a complete explanation of the eligibility requirements, conditions and exclusions in the paragraphs 1.3, 2, 2.1, 2.2, 3.1, 3.2, 3.3, 4.1, 4.2, 4.4, 4.5, 5.2, 6.1, 6.2 and 6.3 of the Agreement.
Brief Summary of Additional Important Terms:
• Finance Charges accrue on unpaid balances while receiving benefits.
• There is a 30-day waiting period from time of enrollment in the Program to be eligible for benefits, except for Death benefits.
• During the Benefit Period your account will be closed and you many not use your Account for any purpose .
Certain Fees and Finance Charges Waived During Benefit Period: During the Benefit Period, the Monthly Program Charge, any Late Payment Fee, Over Limit Fee, and Closed Account Maintenance Fee that may be incurred during the Benefit Period will be waived. If your Annual Membership Fee is billed monthly, the amount due during the Benefit Period will also be waived. If your Annual Membership Fee is billed annually and becomes due during the Benefit Period, assessment of this Fee will be postponed. Assessment of these fees (as applicable) will resume after the Benefit Period ends. During the Benefit Period, periodic finance charges (interest) will continue to accrue on unpaid balances and will be posted to your Account in accordance with your credit card Cardholder Agreement.
Customer Acknowledgement: If you authorized enrollment in the Program, you acknowledge that you have not received all of the terms and conditions of the Program package in advance of the purchase. However, you may for any reason cancel the Program purchase within thirty (30) days of your receipt of the complete terms and conditions of the Program package and your initial Program Charge will be refunded.
The complete Agreement will be sent to you after your credit card account is activated. Please read the Agreement carefully when you receive it.
